Telehealth Consent

What you're agreeing to when you receive care through telehealth.

PLACEHOLDER LEGAL COPY — this page is a structural placeholder. Final language must be drafted and approved by SoliRx's legal and compliance team before publication. Do not treat any text on this page as binding.

What Telehealth Involves

Telehealth uses secure electronic communications to enable a licensed provider to evaluate, diagnose, and treat you remotely. This may include messaging, forms, photos, and — where applicable — live video or phone consultations.

Risks and Benefits

As with in-person care, telehealth carries potential risks, including technical failures and the inherent limitations of remote evaluation, alongside benefits like convenience and faster access to care. A complete risk disclosure will be provided by the clinical and legal teams.

Your Right to Withdraw Consent

You may withdraw consent to telehealth services at any time without affecting your right to future care. Instructions for withdrawing consent will be detailed in the final version of this document.

Emergency Care Limitations

Telehealth through SoliRx is not designed for emergency situations. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room — final placement and formatting of this warning is pending design review.
